To deliver high-quality, person-centred care and support to adults with complex health needs, enabling them to live safely, comfortably, and with dignity. The role involves clinical tasks such as tracheostomy care and PEG feeding, alongside supporting daily living and promoting independence wherever possible.
Key Responsibilities:
- Provide safe, compassionate, and respectful care in line with individual care plans.
- Carry out tracheostomy care, including suctioning, stoma site cleaning, tube changes (where trained and authorised), and emergency management of blocked or dislodged tracheostomy tubes.
- Deliver safe and effective care for clients with spinal cord injuries, including mobility support and pressure area management.
- Administer PEG feeding, including preparation of feeds, maintaining equipment, and monitoring for complications.
- Assist with personal care (washing, dressing, continence care, mobility) while promoting dignity and independence.
- Monitor, record, and report any changes in the individual’s health and wellbeing.
- Recognise and respond promptly to medical emergencies, including tracheostomy or PEG-related complications.
- Support with medication administration in line with training and policies.
- Work collaboratively with healthcare professionals, families, and the wider care team to ensure continuity of care.
- Maintain accurate and up-to-date care records, incident reports, and health observations.
- Adhere to infection control, safeguarding, and health & safety policies at all times.
- Participate in regular training, clinical competency checks, and supervision to maintain up-to-date knowledge and skills.
