About the Company

As a Housing First Co-ordinator, you will lead the delivery of our Housing First programme, ensuring individuals with complex needs are supported to maintain tenancies and improve their quality of life. You will be responsible for coordinating a multi-agency support approach, managing a small team (if applicable), and ensuring that the Housing First principles are embedded in everyday practice.

This is a key role in ensuring that people are offered stable housing and the right support, in the right place, at the right time.

Key Responsibilities:

 

  • Coordinate support for individuals with complex needs who are transitioning into and maintaining housing
  • Work collaboratively with partners including health, social care, substance use services, and housing providers
  • Uphold the principles of Housing First: choice, harm reduction, and person-centred support
  • Monitor and evaluate outcomes to ensure the effectiveness of support interventions
  • Provide guidance and supervision to support staff or peer mentors (if applicable)
  • Maintain accurate records and reporting in line with organisational and funder requirements
