What does this Care Coordinator job involve?

 

 

  • Coordinate rotas and care packages to ensure safe, consistent service delivery.

  • Match staff to service users based on needs, risks, skills and continuity.

  • Respond quickly to sickness, emergencies, hospital discharges and schedule changes.

  • Act as a point of contact for service users, families and professionals, addressing day?to?day queries.

  • Escalate safeguarding concerns, complaints, incidents or changes in need promptly.

  • Provide operational support to care staff, including shift changes and essential updates.

  • Monitor service quality, punctuality, missed calls and operational pressures, taking action where required.

  • Recognise and respond to mental health, dementia or behavioural needs that affect care delivery.

  • Maintain accurate electronic and manual records, including rota updates and service changes.

  • Work in line with GDPR, safeguarding, health and safety and CQC requirements.

  • Previous care experience 

  • Competent IT skills
