About the Company

We are looking for an experienced care assistant/ senior care assistant/ support worker or Field care supervisor to complete these assessments with a good knowledge of all current legislation and experience of leading a team. You will be supporting a team of carers within the community by completing assessments and audits, developing relevant care plans, mentoring and onboarding our new recruits in our fast-growing branch and ensuring that the people we support receive the highest standards of care in line with our company standards and reputation.

What does this Community Facilitator job involve?

  • Carry out regular observations and supervisions of care workers within your area.
  • Carry out new assessments and create care plans for new people we are going to support.
  • Keep all the team aware and up to date on any changes to the people we support reviews.
  • Cover care worker absence, sickness, holidays and provide emergency cover within own hours.
  • Support people to achieve maximum independence.
  • Ensure adequate knowledge and comply with risk assessments.
  • Record complaint incidents accidents as per the reporting policy, keeping all parties involved up to date
