What does this Care Manager job involve?

 

Your Role:

  • Conduct initial care assessments
  • Regularly engage with clients via service reviews
  • Liaise with community professionals such as District Nurses and other healthcare providers to support client care
  • Ensure client care plans are current and fulfil their needs
  • Introduce care professionals to their clients
  • Maintain close contact with care professionals, ensuring they have the necessary information for client care
  • Regularly review with care professionals through supervisions to guarantee they have the necessary training and support
  • Support care professionals in the field with competency assessments
  • Participate in the on-call rota
